Enrolment
Information
The
information collected during the enrolment process is vital
for us to provide high quality care to meet your child/ren's
needs.
You
may choose to download enrolment and e-mail, fax or post
to us. However, we do prefer to have a personal meeting
with you to ascertain your family's and child's likes/dislikes,
health needs and any other information you feel worthwhile
sharing.
When you do come into the centre to enrol please remember
to bring the following:

Copy of immunisation record

Letter from your doctor re conscientious objection to
immunisation |

Centrelink notice re Child Care Benefit Subsidy
|
|

Any relevant court orders
|
When
nominating your Emergency Contact persons please ensure
that it is someone who is available to be contacted to collect
your child if required. We look forward to welcoming you
and your children to our community family at Tweed OOSH
and Vacation Care.
